Think of it as an instruction manual for both the caregiver and the person receiving care. The idea behind it is anyone stepping in to provide care, can read the plan and know exactly what to do.
The primary aim of a care plan is to give the person receiving care as much independence and choice as possible.
What types of Care Plan are there?
- General Care Plans: These are your basic, one-size-fits-most plans covering essential daily tasks, such as grooming, meals and medication.
- Individualised Care Plans: Tailor-made for each patient. These get down to the nitty-gritty, talking about personal habits, daily routines, and even specific dietary needs.

What should a Care Plan include?
A well-crafted care plan should be as unique as the person it's designed for. However, there are some general elements you shouldn’t overlook:
- Client Profile: Name, contact information, personal history, medical history – the basics!
- Diagnosis: What are the client’s care needs? Are they physical, emotional or social?
- Goals and Objectives: What are you hoping to achieve with the care provided? This could be as simple as maintaining a certain level of mobility, or as complex as managing a medical condition.
- Interventions and Actions: The ‘how’ of the plan. This is where you note the type of care and how often it’s needed.
- Medication: Any meds the client is on, dosage and timing should all be in here.
- Schedule: When will care be provided and by who exactly? Slot it all in.
- Tasks: Break down the daily or weekly tasks that need to be performed. Be specific!
- Risk Assessment: Health and safety should be priority numero uno. Spell out potential risks and how to minimise them.
- Review Dates: Health is an ever changing landscape. Plans shouldn’t be set in stone, and you’ll need to regularly review and tweak the plan.
